Home Health Services
Skilled Nursing Care
Our registered nurses and licensed practical nurses are highly skilled, licensed and certified through appropriate agencies and receive continuing educational instruction and training.
Care is personalized for each client with focus on both short- and long-term care goals and concerns. Nursing team members work closely with the client’s medical team to ensure that doctor’s directions and orders are carried out properly.
Horizon nurses also complete a monthly head-to-toe nursing assessment for each client to evaluate changes in current medical conditions and identify any new medical-related illness or condition.
Physical, Speech & Occupational Therapy
We are dedicated to helping clients achieve their greatest possible level of independence by working closely with independent physical, occupational and speech therapists to provide doctor-ordered therapy. Our skilled nurses provide the most appropriate services by working as a team with the client, their family and doctor.
Tracheotomy Care
A lack of or improper tracheotomy care can lead to severe infection and complications. Horizon nurses are trained in performing proper tracheotomy care that includes suctioning, decannulation and trach changes. Our nurses monitor clients for aspiration, blockages, bleeding, respiratory distress, collapsed lungs, skin problems and infections.
IV Medications/Injections
We follow stringent guidelines for providing safe intravenous (IV) medication injections to clients. Our licensed nurses are trained and experienced to provide all IV medication delivery methods. Patient identification and verifying proper drug, dose and time are essential in achieving IV medication injection safety.
Medication Management
Seniors take an estimated minimum of four to six prescription medications daily. Managing their medications is critical to maintaining and improving health and wellness. We focus on safety and quality for storage, ordering, dispensing, administration and monitoring of medications.
Our staff picks up medications at the pharmacy of the client’s choice, administers medications, and prepares pillbox organizers and insulin for home use to avoid errors in medication selection and dosage. Our nurses also educate clients about possible side effects and monitor them for adverse medication reactions.
Chronic Disease Care
We ensure the doctor’s care plan is followed and communicate changes in the client’s condition to doctor regularly. Our nursing staff provides daily training sessions that focus on stress reduction, medication management, and the improvement of mental and physical health.
Wound Care
Healing and avoiding further complications is done by proper assessment and care of wounds.
Urinary Catheter Care
Our procedures are essential in preventing urinary tract infections and complications. Our nursing staff provides catheter changes, flushes, measurement of output and obtaining doctor-ordered specimens.
Restorative Therapy
Disease and medical conditions can make it difficult for clients to care for themselves or to continue living independently.
For instance, many stroke victims suffer from paralysis, making it difficult or impossible for them to perform daily-living tasks.
