What We Do

When a loved one is sick or injured, families are faced with very difficult decisions when caring for their elderly loved one.  Before Adult Day Health Care, families were faced with placing their elderly loved one in a nursing facility or leaving work and losing vital family income.  Adult Day Health Care gives individuals and families an alternative in their health care decisions and keeps families together while insuring the best possible quality of life for its clients. 

Horizon Adult Health Care provides a safe, comfortable, and homelike atmosphere for the frail elderly and disabled during the daytime hours.   H

All of our 12 centers are staffed with licensed nurses, state registered nurses aides, and case managers.  Each center provides nutritious meals and snacks, nursing care, individual care plans, age appropriate games and activities, door-to-door transportation, and so much more.
